VISITOR HEALTH SCREENING FORM

To reduce the risk of COVID-19 exposure to HORST Engineering and Sterling Machine Company employees, all visitors are required to complete all screening questions.

This form must be submitted back to HORST Engineering five business days prior to any scheduled visit.

All visitors will also be asked to fill this form out again upon arrival and may be subject to temperature testing. Any visitors answering yes to any of these questions will not be approved for access to any of HORST Engineering’s facilities.
